Polk County Housing Continuum

Mission: To identify the state of homeless and affordable housing in Polk County and to create a coordinated plan that addresses the economic and social challenges that produce homelessness and lack of affordable housing. This includes fostering open communication and facilitating collaboration among existing affordable housing delivery systems.

Affordable Housing, according to Housing and Urban Development (HUD), housing at a cost that does not exceed 30% of the household's gross income

The Housing Continuum’s membership consists of agencies, organizations, and groups that provide services that address the priorities of the Housing Continuum. Those priorities consist of affordable housing opportunities including:

Permanent Supportive Housing: Homeownership & Rental: Rental or owner occupied households that require either financial assistance to make their housing affordable and/or housing support services to assure family stability and self-sufficiency.
Transitional Housing: Housing, coupled with support services such as case management and counseling, which strengthens the individual/family and facilitates movement to permanent housing within two years.
Emergency Shelter Housing: Facilities with overnight accommodations whose primary purpose is to provide temporary shelter for those that are homeless, or for a specific population of homeless persons.
Street Outreach: A multifaceted approach to inform people without stable housing how to access available services and basic needs such as housing, healthcare, and food.
